Section 5 - F5 Farm is a working farm and agritourism environment. F5 Farm does not evaluate, monitor, or manage participant allergies, medical conditions, physical limitations, sensitivities, or other personal circumstances through this form. The parent or legal guardian is solely responsible for deciding whether the minor participant is able to safely attend and participate in a farm environment, including around animals, hay, straw, dust, dirt, mud, insects, weather, uneven ground, and other ordinary farm conditions. If the parent or legal guardian believes the minor participant requires any medication, special precautions, heightened supervision, or other support, those matters are solely the responsibility of the parent or legal guardian unless expressly agreed otherwise in writing by F5 Farm.*

I understand that it is my responsibility to determine whether the minor participant can safely attend and participate in a farm environment and that F5 Farm is not responsible for evaluating or accommodating allergies, medical conditions, physical limitations, or other personal circumstances through this form.

Section 7 - Assumption of Risks*

I understand and acknowledge that entry onto a farm and participation in agritourism and animal-related activities involve inherent and other risks, including but not limited to: contact with animals, including cattle and other livestock; unpredictable animal movement or behavior, including bumping, stepping, kicking, or sudden movement; uneven ground, mud, gravel, manure, hay, straw, slippery areas, holes, ruts, and natural terrain; fences, gates, barns, pens, feeders, tools, equipment, and other farm structures or objects; insects, bees, wasps, ticks, spiders, wildlife, dust, dander, mold, allergens, feed, and bedding; weather and environmental conditions, including heat, cold, wind, rain, lightning, and ice; the actions or inactions of other participants, guests, children, staff, volunteers, contractors, or third parties. I understand that these risks may result in bodily injury, illness, allergic reaction, property damage, emotional distress, permanent disability, or death.

I have read and understand the risks described above and voluntarily permit the minor named in this form to participate.

Section 8 - Permission to participate in activities at F5 Farm LLC.*

On behalf of myself and the minor participant, I knowingly and voluntarily permit the minor to enter the property and participate in activities at F5 Farm. I understand and accept the risks associated with participation in a farm and agritourism environment, to the fullest extent permitted by law.

I give permission for the minor to participate in farm activities and acknowledge the risks described in this form.

Section 9 - Clothing & Footwear Requirements*

I understand and agree that appropriate clothing and footwear are required for the minor participant. At a minimum: closed-toe shoes are required; open-toed shoes are not permitted; high heels are not permitted; clothing must be practical and appropriate for a farm environment and weather conditions. I understand that failure to comply may result in denial of entry or removal from the property.

Section 10 — Photography, Video, and Media Release: F5 Farm may photograph or record activities for promotional purposes including website, social media, and marketing materials.*

I understand and agree that photographs, video recordings, and other media may be taken during visits, events, and activities at F5 Farm. For valuable consideration, including being permitted to enter the property and participate in activities, I grant to F5 Farm LLC d/b/a F5 Farm the right to photograph, record, use, reproduce, publish, display, distribute, edit, and otherwise use the minor participant’s name, image, likeness, voice, and appearance in any photograph, video, digital media, or promotional material, in any format or medium now known or later developed, for lawful business purposes, including but not limited to: social media posts; website content, including F5farmlife.com; advertising and promotions; print marketing materials; event promotion; and other farm-related marketing purposes. I understand and agree that no compensation will be owed for such use, that I have no right to inspect or approve the final use of any media, and that F5 Farm LLC will own all rights, title, and interest in such media created by or for it.

I grant permission for the minor participant’s image, likeness, and appearance to be used for lawful business and promotional purposes as described above.

Section 11 - Release of Liability*

To the fullest extent permitted by law, I, on behalf of myself, the minor participant, and our heirs, assigns, personal representatives, and next of kin, release, waive, discharge, and covenant not to sue F5 Farm LLC d/b/a F5 Farm, and its owners, members, managers, officers, employees, volunteers, agents, representatives, contractors, affiliated entities, successors, and assigns, from and against any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or relating to the minor’s entry onto the property, participation in activities, interaction with animals, use of the farm premises, or any injury, illness, death, or property damage sustained by the minor, to the fullest extent permitted by law.

I have read and agree to the release of liability above on behalf of myself and the minor participant, to the fullest extent permitted by law.

Section 12 - Illinois Agritourism Acknowledgment*

I understand that F5 Farm is an agritourism operation and that Illinois law may limit an agritourism operator’s liability for injuries or death resulting from the inherent risks of agritourism activities, subject to applicable legal requirements and exceptions. I understand that warning signage may be posted at or near the entrance and activity areas in accordance with Illinois law.

I acknowledge that this activity takes place in an agritourism setting and that inherent risks are involved.

Section 13 — Farm Rules & Safety Compliance.*

I agree that the minor participant will follow all posted rules, written instructions, verbal directions, safety requirements, and directions given by F5 Farm and its staff or representatives at all times.

I understand that F5 Farm reserves the right to deny entry, refuse participation, or remove any person from the property for unsafe, reckless, disruptive, abusive, or non-compliant behavior, including mistreatment of animals or entering unauthorized areas.

I agree that the minor participant and accompanying adults will follow all farm rules, instructions, and safety requirements.

Section 14 - Supervision of Minor*

I understand that minors must remain under appropriate supervision at all times, unless F5 Farm expressly states otherwise for a specific activity.

I accept responsibility for appropriate supervision of the minor participant and understand that unsafe behavior may result in removal or denied participation.

Section 15 - Medical Treatment Authorization*

I understand that F5 Farm does not provide medical care and may not have medical personnel on site. In the event of an emergency, I authorize F5 Farm to seek emergency assistance or medical treatment for the minor if reasonably necessary. I understand that I am solely responsible for any resulting medical costs except as otherwise required by law.

I authorize emergency medical assistance for the minor if reasonably necessary and accept responsibility for related costs, except as otherwise required by law.

Section 16 - 24-Hour Signature Requirement / No Entry if Unsigned*

I understand that this form must be fully completed and signed no fewer than 24 hours prior to arrival, unless otherwise expressly approved by F5 Farm. If this form is not signed and received in time, the minor participant will not be permitted to enter the farm or participate.

I understand that this signed form is required at least 24 hours before arrival and that no entry will be permitted if it is not completed in time.

Section 17 - Electronic Signature and Final Certification*

By signing below, I certify that I have carefully read this form, understand its contents, understand that it includes an assumption of risk, release of liability, rules acknowledgment, and media release, and sign it knowingly and voluntarily as the parent or legal guardian of the minor participant listed above.

Agritourism Warning Notice

This form must be completed and signed by the parent or legal guardian of any minor participant at least 24 hours prior to arrival at F5 Farm. If this form is not signed and received in time, the minor will not be permitted to enter or participate.

This form applies to visits, events, experiences, animal interactions, photo opportunities, and other activities taking place at F5 Farm LLC d/b/a F5 Farm.

WARNING:
Under Illinois law, there is no liability for an injury or death of a participant in an agritourism activity conducted at this agritourism location if such injury or death results from the inherent risks of the agritourism activity. Inherent risks of agritourism activities include, among others, risks of injury inherent to land, equipment, and animals, as well as the potential for participants to act in a negligent manner that may contribute to injury or death. You are assuming the risk of participating in this agritourism activity.
