Section 4 — Farm Safety Acknowledgement*

I acknowledge that farms contain inherent risks including animals, uneven ground, equipment, and natural hazards. I agree to follow all posted farm rules and staff instructions.

Section 4A - Footwear Policy*

I understand that appropriate footwear is required. Open-toed shoes, sandals, and high heels are not permitted in animal areas.

Section 5A. Animal Unpredictability / Inherent Risk Clause: I understand and acknowledge that farm animals, including but not limited to cattle, may act in sudden or unpredictable ways that may result in property damage, personal injury, illness, or death. I further understand that inherent risks of farm and agritourism activities include, but are not limited to, animal behavior, uneven ground, mud, holes, gates, fencing, changing weather, insects, allergens, farm equipment, and the ordinary dangers of a working farm environment. I voluntarily assume these inherent risks for myself and for any minor participant(s) listed in this waiver.*

Section 5B. Spectator / Proximity Risk Clause: I understand that risks may exist whether I am directly participating in an activity or merely observing, standing near, photographing, feeding, or otherwise being present in or around animal areas, activity areas, barns, fenced spaces, and farm grounds. *

I acknowledge that injury may occur from the actions of animals, other guests, crowd movement, gates or barriers, failure to follow instructions, or conditions of the farm premises, and I voluntarily assume those risks.

Section 5C— Assumption of Risk and Liability Waiver: I acknowledge that participation in farm activities involves inherent risks including, but not limited to, interaction with animals, uneven terrain, farm equipment, weather conditions, and other natural hazards. I voluntarily assume all risks associated with visiting and participating in activities at F5 Farm LLC.*

I have carefully read Section 5 in its entirety and release of liability, fully understand its contents, and voluntarily agree to its terms on behalf of myself and any minor participants listed in this submission.

Section 6 — Photo & Media Release: F5 Farm may photograph or record activities for promotional purposes including website, social media, and marketing materials.*

I grant permission for F5 Farm LLC to use photographs or video taken during my visit for promotional and marketing purposes.

Section 7 — Agreement for All Current and Future Visits*

I understand this waiver applies to myself and all listed participants for current and future visits to F5 Farm unless revoked in writing. This waiver is valid for 1 year from the date of signature.

Minor participant Clause*

I certify that I am the parent or legal guardian of any minor listed in this waiver and that I have the authority to sign this waiver on their behalf.

I did not list any minor participants on this form.
